PATIENT INFORMATION Each Trust has a variety of patient literature which is usually
made up of information from different sources. Although helpful,
too many leaflets and brochures can be confusing, and the Cardiac
Rehabilitation Network Group wanted a sector-wide patient information
leaflet, that captured the key points necessary for a generic
patient information brochure.
A brochure has now been printed
and distributed to each Cardiac Rehab team across North West
London. The brochure is available in a variety of different languages
for distribution across the various organisations.

The H2H User Forum is introducing 'home packs' to all organisations
across North West London. The 'home pack' concept was developed
by West Middlesex University Hospital and aims to provide lifestyle
information to patients following e.g. heart surgery, pacemaker
or angioplasty procedures. The packs incorporate advice on
everything from healthy eating to rehabilitation and physical
activity. The packs also give patients invaluable working knowledge
about living with their condition. It is expected that these
'home packs' will soon be available within every hospital in
the area.

What are patient voices?
Patient voices are written or spoken short stories of ordinary patients about their experiences with healthcare, how they dealt with their disease and what impact the disease had on their lives.
Patient voices can be found on several websites.
Why publish patient voices?
The aim to publicise patient voices on the website is to make managers aware, who are devising and implementing strategies in health care, as well as the professionals and clinicians directly involved in care, so that they may carry out their duties in a more informed and compassionate manner.
It may also give support to those who have had a stroke or who have cardiac disease and their families.
