Heart Failure Services in North West

This guideline has been developed by clinical
teams, managers and service users across primary, secondary and
tertiary care; and outlines best practice for health professionals'
caring for individuals who have, or who are suspected of having
heart failure.
How heart failure affects the population of North West London is presented
within the document and local health communities should review their existing
service provision for people who have heart failure against this guideline
together with key stakeholders involved in the development, delivery and procurement
of heart failure services.

West London Heart Failure Directory of Services and Resources -
The Directory of Services and Resources was developed to provide
an informative reference guide of heart failure services and resources
across North West London. It is aimed at the health care professional
but patients, carers and their families will find it a useful source
of information for their locality. The organisations themselves have provided us with their details
and we have taken every care to check that each entry is correct
at the time of publication. We hope they will provide you with
a good service but we cannot make personal recommendations or guarantees
of quality.
This directory is a web-based service and will be updated as regularly as possible.

Making the Best Use of Hospital Beds - In June 2007 the Heart Improvement Programme invited Networks to bid to participate in a National initiative - 'Making best use of hospital beds'. The aim being to work with a number of Networks to identify practice that will make best use of in-patient beds by reducing the number of avoidable emergency bed days, length of stay and developing alternative models of care. This document is the final report of the 'Early discharge project for heart failure patients' and outlines the project aims, objectives and outcomes.
Heart failure Personal Notebook Evaluation - In December 2007 the Network published a personal notebook for heart failure patients. The concept behind the notebook was to empower patients with heart failure to effectively manage their long-term condition and encourage more effective communication between patients and healthcare professionals. This document assesses the notebook from both the patient and heart failure nurses perspective.
