Acute Hospital Portfolio Review Acute Hospital Portfolio Review 2005/2006 - The Acute Hospitals Portfolio Review is run via the Healthcare Commission on an annual basis, to inform the annual healthcheck process. An acute hospital portfilio around the mangement of heart failure admissions was conducted last year. Heart Failure Blueprint

A Blueprint for Excellence in Heart Failure Services in North West London - this guideline has been developed by clinical teams, managers and service users across primary, secondary and tertiary care; and outlines best practice for health professionals' caring for individuals who have, or who are suspected of having heart failure. The document provides recommendations on the appropriate treatment and care of patients that are most effective in confirming a diagnosis, provision of prognostic information, influencing therapy and chronic disease management.

How heart failure affects the population of North West London is presented within the document and local health communities should review their existing service provision for people who have heart failure against this guideline together with key stakeholders involved in the development, delivery and procurement of heart failure services.

Heat Failure Assessment Doc 05 Results of the Heart Failure Assessment 2005 - An initial scoping exercise was completed to gain baseline understanding of heart failure services across NW London and to inform priorities for the work programme Sub groups. As part of the DOAS project another scoping exercise was completed in November 2005 following the same format as 2004, with an additional section on IT systems. This document provides an overview of the changes and developments within heart failure services in NWL over the last year. Please click on the icon to view the document.

North West London Heart Failure Directory of Services and Resources - The Directory of Services and Resources was developed to provide an informative reference guide of heart failure services and resources across North West London. It is aimed at the health care professional but patients, carers and their families will find it a useful source of information for their locality. The directory is based on the West London Cancer Network directory, providing advice, support and relevant information for the heart failure patient. The directory has been arranged into sections and aims to provide specific information available within each locality across North West London. You may only want to read the section that is of special interest to you and not read the whole directory. We have tried to include details of all local statutory services and voluntary organisations that offer services in the hope to meet some of the needs of people living with heart failure and in particular chronic heart failure. The organisations themselves have provided us with their details and we have taken every care to check that each entry is correct at the time of publication. We hope they will provide you with a good service but we cannot make personal recommendations or guarantees of quality.
